REQUIRED FORMS

Required forms and fees must be turned in to the Athletic Office before the student starts practice. These forms may be obtained from the Athletic Office.  Once they are signed by the athlete and the parent in all required areas, a copy is kept on file in the Athletic Office.

INSURANCE

All athletic participants must provide their own insurance. The school district does not provide individual insurance. School insurance can be purchased and applications are available in the athletic office.

FEES

To be paid in Fees Office. $50.00 per sport. Maximum of $100.00 per family each individual sport season (fall, winter, or spring). If student is on Reduced Lunch Plan, he/she will pay $25.00 per person per sport; maximum of $50.00 per family each sport season (fall, winter, or spring). If on Free Lunch Plan, he/she will pay $0. All student athletes must purchase an A.S.B. card for $25.00. This card allows students to participate in sports, will get them into all home games, (not playoffs) and will give discounts to some dances.

STUDENTS WISHING TO RIDE HOME WITH OTHERS

As per North Wasco School District 21 Board Policy, No student is allowed to drive himself/herself or others to scheduled school events.

All students are to ride the bus or van with the team to away athletic events. They may ride home with a parent/guardian only when signed out by the parent/guardian at the away event. See the coach for the sign-out sheet. The coach is not allowed to release a student if he/she is not given authorization by the Athletic Office prior to leaving for an athletic event.

If a student wants to ride home with someone other than their parent or legal guardian, the driver must have Bodily Injury and Liability Insurance and must fill out a North Wasco County School Dist. 21 Proof of Vehicle Liability Insurance form which is available in the Athletic Office or may be downloaded by clicking the link below. This information must be sent or FAXED to North Wasco School District 21 office, FAX # 541-506-3422, Phone # 541-506-3420 at least one day before scheduled event. The District office will update the Athletic Office with this information. This information is deleted when the policy expires (every 6 to 12 months generally). It is the responsibility of each driver to make sure this information is up to date. The parent of the student who is to ride home with someone else must present in writing authorization for the student to ride with authorized person. Please give this to Diana in the Athletic Office one day prior to scheduled event. This is a School District #21 Policy and our coaches have no choice but to follow it. It is a liability issue and any person transporting students could be held liable.
